San Ysidro Health Center, Inc. – San Diego, CA

Under the direct supervision of the EIS Program Supervisor, and the technical supervision of the Case Management Supervisor, the Case Manager is responsible for assisting people with HIV/AIDS access the continuum of HIV care services. The Case Manager will be responsible for providing direct services, and coordinating services for clients who are HIV+ and conducting a comprehensive uniform assessment in the following areas: health status, medical care and provider, activities of daily living and mental health status. The Case manager will be responsible for maintaining comprehensive files for each client and developing a care plan based on client needs. Will represents Our Place in the community, interacting and networking with community service agencies that provide services to HIV positive individuals and their families. Cultivates and maintains communication and networking with multi-ethnic organizations in the HIV/AIDS community. Serves as a role model for cultural and linguistic competency. Maintain client confidentiality at all levels.

Essential Functions of the Job

  • Develop excellence in case management services which will enrich and enhance the quality of life, dignity and respect of people infected and/or affected by HIV/AIDS.
  • Conduct a comprehensive assessment and administer an acuity scale to determine the medical, psychosocial, and social service needs of HIV+ clients. Assessment is completed at intake and every four months at minimum. Face to face and phone contact with clients is completed at minimum of once monthly.
  • Ensure clients are enrolled in medical care and receive appropriate referrals to needed services as established in case plan. Services include (but are not limited to): medication assistance, housing, mental health counseling, substance abuse counseling, legal services, etc.
  • Advocate for clients and assist them in the procurement of benefits and services. Provide necessary information, referrals for established care plans and advocate for client needs.
  • Maintain a caseload of 30-50 clients (caseload may be below 30 based on acuity level of caseload and with prior approval from Case Manager Supervisor).
  • Responsible for entering all required client related data into ARIES within 2 business days of completing the service.
  • Timely professional maintenance of records and documentation, including progress notes, treatment plans and ongoing evaluation of client progress, consistent with agency expectations and ethical standards.
  • Follow all confidentiality and HIPPA guidelines and protocols.
  • Increase relationships and communication with other San Ysidro Health Center programs and locations.
  • Represent San Ysidro Health Center and Our Place to other organizations and community groups. Network with other AIDS service providers.
  • Provide Spanish language interpretation as required to assist clients in meeting case plan.

Additional Duties and Responsibilities

  • Provide AIDS Drug Assistance Program (ADAP) and Ryan White enrollment to own caseload and ensure all eligible clients complete the ADAP enrollment process.
  • Assists the Case Manager Supervisor in monitoring the quality assurance and evaluation process ensuring contract compliance and outcome and process objectives for clients and their families are met.
  • Assists in the preparation of monthly narrative and statistical reports covering progress toward meeting outcome and process objectives established in the Our Place grant proposal.
  • Support and participate in activities and group events organized by HIV Department including but no limited to AIDS Walk, San Diego LGBT Pride Parade, and Clients’ Holiday Event.
  • Participate in all multi-disciplinary team meetings, staff meetings, and selective staff development trainings, as directed.
  • Maintain a flexible schedule to allow for some weekend work as well as emergency coverage of shifts to meet the needs/demands of clients and related activities and events.
  • Coordinate and post weekly schedules, advising Program Supervisor with any changes or time-off requests.
  • Responsible for punching in daily and being compliant with SYHC policies and procedures.
  • Perform other related activities as required or assigned by the Part C Program Manager, Case Manager Supervisor.

Job Qualifications

Education:

  • Required: Bachelor’s Degree in human services or related field or high school degree with 3 years of experience working in non- profit social service agency.

Experience :

  • Required:
    • Prior case management or counseling experience and experience in group facilitation and educational instruction preferred.
    • Three (3) years of experience working in human or social services. Experience with and sensitivity to LGBT-related cultural concerns.
    • Knowledge of health and psychosocial issues concomitant to HIV disease.
  • Preferred: Previous HIV field experiences a plus.
  • Certifications and Licenses :

Required: Current valid California driver’s license and access to insured vehicle required.

Verbal and Written Skills to Perform the Job :

Bilingual English/Spanish required, with excellent oral and written communication skills. Communicate clearly and concisely, both orally and in writing.

Skills Required to Perform the Job :

Exercise tact, objectivity, sensitivity, strategy and judgment in dealing with a variety of people with a variety of co-occurring disorders. Organize, set priorities and exercise sound independent judgment within areas of responsibility. Establish and maintain effective working relationships with professional staff, clients, family members, and the public.

Equipment Used:

Computer literacy required, proficiency in MS Office (Word, Excel, Outlook and PowerPoint), projectors, various software including ARIES, and Microsoft office.

Working Conditions:

Physical Activities standing, walking, sitting and lifting 25 lb. Working Traits: travel to other facilities. May work evenings and/or weekends.

Our Place is a Coordinated Services Center located at 286 Euclid Avenue, Suite 309, San Diego, CA 92114, and is dedicated to improving the health and quality of life of people living with HIV / AIDS, specifically serving those living within the Southeastern and Central regions of San Diego.

Programs and Services
Our Place is a one stop shop for HIV services and information offered in a warm, home-like setting which promotes social interaction and encourages recreational activities.  Our Place offers assistance and linkages to HIV medical care and treatment, as well as providing application assistance to a variety of programs including Ryan White and ADAP. HIV Medical Care
Our Place offers access to HIV treatment at the King-Chavez Health Center, located at 950 S. Euclid Ave in San Diego, which includes treatment education, dental and pharmacy services. Our HIV specialists provide services Monday through Friday, 8:30 AM – 5:00 PM.

Case Management
The Our Place Case Management Team assists in navigating the HIV care system, including linkages to HIV medical care, medications, housing, food, transportation and other social services.

Mental Health and Substance Abuse Counseling
Our Place coordinates Mental Health Counseling Services which provides individual, couples, and family counseling for HIV-positive clients and their families on Wednesdays 12:30 – 4:30 PM.  Substance Abuse Counseling Services include relapse prevention and early recovery therapy, available on Wednesdays from 10:00 AM – 12:00 PM.

HIV Prevention & Testing Services
Our Place Prevention Services strive to reduce the spread of HIV through individual and group services, as well as access to HIV testing with same-day results.

Support Group

Our Place’s, “Help 4 All,” is conducted in English and meets every first and third Friday from 12:00 – 2:00 PM.  These sessions are open to men and women living with HIV / AIDS, and their families.

Educational Presentations highlight various useful & informative messages, tools and support for our attendees and their guests.  These programs are offered on the second and fourth Wednesdays, each month, from 12:00 – 2:00 PM and are open to men and women living with HIV / AIDS, as well as their families.

Movie Fridays is a program which offers a comforting, casual, social environment to attendees, featuring movies-on-demand every Friday, from 9:00 AM to 5:00 PM.
