Under the direction of the Medical Case Manager Supervisor, the CM Patient Navigator conducts outreach to identify qualifying individuals for linkage to medical case management, Covered California, Medi-Cal and/or Ryan White. The CM Patient Navigator directly provides health education and referrals to HIV positive patients in the agency’s caseload. Maintains program records and performs a wide variety of patient care activities as needed. Represents SYHC HIV Department’s programs and services in the community, interacting and networking with community service agencies that provide services to HIV positive individuals and their families. Cultivates and maintains communication and networking with multi-ethnic organizations in the HIV/AIDS community. Maintains confidentiality at all levels.

This is a temporary part-time position.

  1.  Coordinate and maintain linkages with medical case management to identify HIV positive clients who qualify for Covered California, Medi-Cal and/or Ryan White.
  2. Assist clients in accessing services by linking them to care, following up, and ensuring clients are linked to and complete their medical appointments.
  3. Link clients to medical and support services through information, referral, assistance with filling out and understanding necessary forms, and other case tasks.
  4. Complete outreach to key agencies in the region to facilitate direct cross referrals by providing a point of contact and general HIV services information.
  5. Assist clients in the procurement of benefits and services. Advocate for client needs. Provide support, advocacy, information, resources and referral.
  6. Provide follow up to clients that have failed HIV medical and support services appointments, and assist them back into care.
  7. Maintains a caseload as assigned by contractual obligations. Perform tasks related to caseload, i.e., correspondence, reports, maintenance of case/group records, client service evaluations, etc.
  8. Perform tasks related to agency-wide data (daily and monthly activity reports; other monthly reports) in a timely fashion, meeting all expected deadlines.
  9. Participate in selective multi-disciplinary team and Center staff meetings; agency staff meetings, and selective staff development trainings.
  10. Timely professional maintenance of records and documentation, including progress notes and ongoing evaluation of client progress, consistent with agency expectations and ethical standards.
  11. Assists HIV Department in the promotion and implementation of client activities, educational presentations, and special events. Supports and participates in activities and group events organized by HIV Department, specially NHTD, NLAAD, NBAAD, AIDS Walk, LGBT Pride, and Clients’ Holiday Event.
  12. Represent San Ysidro Health Center and HIV Department to other organizations and community groups. Network with other SYHC programs and HIV/AIDS service providers. Increase relationships and communication with other San Ysidro Health Center programs and locations.
  13. Represent San Ysidro Health Center and HIV Department to other organizations and community groups. Network with other AIDS service providers.
  14. Maintain established departmental policies and procedures, objectives, quality assurance program safety, environmental and infection control standards.
  15. Ensure all records and activities are conducted in accordance with SYHC Quality Assurance Policy and Procedures.
  16. Perform other related activities as required or assigned.

Job Qualifications

Education:
·
Required: High School diploma or GED Equivalent.
· Preferred: Bachelor’s degree in social/human field.

Certifications/Licenses:
·
Required: Current driver’s license and car insurance coverage . Treatment Adherence certification from the California Statewide Training & Education Program, a plus.

Experience:
·
Required: Experience working in human or social services. Experience with and sensitivity to LGBT-related cultural concerns and the psychosocial issues concomitant to HIV disease. Previous HIV field experiences a plus.

Verbal and Written Skills to Perform the Job:

Required: Bilingual (English/Spanish)
· Required: Excellent written and verbal communication skills.

Technical Knowledge and Skills Required to Perform the Job:

Demonstrated ability to be culturally sensitive and appropriate in working with minority communities. Demonstrate respect for diversity. Knowledge of HIV/AIDS and its physical, psychological & emotional effects on individuals and family units. Knowledge of HIV treatment, and health and social issues for people living with HIV/AIDS. Excellent interpersonal skills. Exercise tact, objectivity, sensitivity, strategy and judgment in dealing with a variety of people with a variety of co-occurring disorders. Organize, set priorities and exercise sound independent judgment within areas of responsibility. Establish and maintain effective working relationships with professional staff, clients, family members, and the public.

Working Conditions and Physical Requirements :

Physical Activities standing, walking, sitting and lifting 25 lb. Working Traits: travel to other facilities. May work evenings and/or weekends.
