CLINICAL SUPERVISOR

Reports to: Executive Director

Supervises: Program Staff

Classification: Regular Full-Time, Exempt

POSITION SUMMARY:  The Clinical is responsible for managing, supervising, and providing case management for AHA/SF’s programs including, but not limited to, housing counseling, emergency housing, financial services, and job training and supportive employment. This position will ensure documentation of individual case notes and achievement of program outcomes, and assist with reporting to funders. The Clinical Supervisor will conduct community outreach to drive awareness and engagement with services, and is responsible for assisting the Executive Director in evaluating and improving programming.

KEY RESPONSIBILITIES

  1. Provide leadership, clinical supervision, and support to 7+ staff members, interns, and volunteers who serve approximately 1,000 members of the LGBTQ or HIV+ communities annually.
  2. Plan and implement overall program activities in accordance with AHA mission and goals.
  3. Provide support for program development and research. Identify, develop and implement opportunities, participate in creating strategic plans for growth.
  4. Implement long-term goals and objectives to achieve successful outcomes.
  5. Implement program evaluation framework to assess strengths and areas for improvement.
  6. Develop and maintain excellent relationships with partner agencies and communicate effectively with stakeholders.
  7. With the Executive Director, make public presentations on behalf of the program to key community stakeholders, including publicly elected officials.
  8. Develop and co-facilitate community workshops on affordable housing, financial literacy, life skills development, and other topic areas.
  9. Ensure that activities operate within policies and procedures and professional standards.
  10. Enhance forms and systems to document program activities.
  11. Monitor activities to ensure effective and efficient service delivery while staying within budgetary limits.
  12. Recruit, train and supervise staff and volunteers and manage performance.
  13. Review and approve Individual Housing Plans created by program staff.
  14. Oversee quarterly chart audits and track chart compliance.
  15. Ensure workload is distributed equitably by monitoring overtime and task assignments.
  16. Write reports as needed and conduct annual program evaluation.
  17. Provide case management utilizing agency Intake and Assessment database to develop individual housing plans; ensure housing plan implementation and ongoing support services and counseling.
  18. Other duties as assigned.
  19. This job requires working occasional evenings and weekends.

REQUIRED QUALIFICATIONS AND SKILLS

  1. Master’s degree in social work, public health, or other related field; LCSW, MFT or PsyD. strongly preferred.
  2. At least three years experience in nonprofit program supervision; housing experience preferred.
  3. Knowledge of San Francisco specialized programs, including housing, homelessness, HIV/AIDS, substance abuse, mental health, and LGBT services.
  4. Ability to multi-task and thrive in a high-energy, fast-paced environment.
  5. Proactive, positive approach to problem solving; critical thinking skills.
  6. Computer and office skills; strong math skills; comfort with File Maker Pro, Excel, Word, MS Office, databases.
  7. Demonstrates the necessary attitude, knowledge, and skills to deliver services with cultural affinity and to work effectively with diverse populations, especially people with HIV/AIDS, all genders and orientations, and those experiencing crisis, homelessness, poverty, disability, or behavioral health needs.
  8. A thorough understanding, enthusiasm, and commitment to agency mission and activist principles to achieve social change and economic justice.
  9. Bilingual English/Spanish a plus.
  10. Ability to work evenings and weekends as needed.
  11. Capacity to develop and maintain open, friendly, professional interpersonal relationships with internal and external colleagues.

HOUSING ADVOCATE III

Reports to: Clinical Supervisor

Classification: Regular Full-Time, Non-exempt

POSITION SUMMARY:  Under the direction of the Clinical Supervisor, the Housing Advocate is primarily responsible for ensuring smooth, timely and accurate intake of our members; using established standards, business rules, and policies as guidance, the AHA Housing Advocate will act as the AHA’s first point of contact for all new and existing members. Additionally, the AHA Housing Advocate will interface with all colleagues on the AHA Team, maintain member confidentiality, and, promote a sense of pride and professionalism at all times.

KEY RESPONSIBILITIES

  1. Conduct screening interviews at AHA with applicants seeking admission to AHA’s Programs and Services. Confer with referral agents and other internal AHA personnel concerning prospective members, and requesting written background information when necessary.
  2. Process admitted members via orienting them to AHA’s Programs and Services. Explaining applicable rules and regulations. Obtaining necessary signatures on formal agreements. Initiating case record folders.
  3. Refer ineligible applicants to other, more appropriate community agencies and institutions.
  4. Provide case management with the members through the Drop-In Clinic and develop individualized housing plans, which includes plans for increasing income, housing search, budget assistance, job training and enhancement, and, landlord advocacy.
  5. Ensure accurate intake assessment is completed for each member, with an emphasis on housing, psychiatric, family, employment, benefits and recovery needs.
  6. Coordinate new and existing services for members through outside service providers.
  7. Provide ongoing support services by monitoring members individualized housing plan with appropriate goals, objectives and timelines.
  8. Provide tenants rights counseling and landlord mediation services.
  9. Maintain all necessary client information including immediately & accurately entering case notes, enter service activities and outcomes on a weekly basis.
  10. Work occasional evenings and weekends as needed for programmatic activities.
  11. Other responsibilities as assigned to support specific AHA needs.
  12. Set appropriate boundaries with members; observe member confidentiality protocols as needed and required for operation of Member Programs and Services.
  13. Maintain ongoing contact with current and prospective referral sources, which is sufficient to ensure referrals at an adequate level to maintain program integrity.
  14. Ensure the overall maintenance and security of open and closed files as assigned.
  15. Prepare and maintain statistical reports required by AHA policy and procedure. Preparation of the weekly “Case Management and Staff Meeting” report as assigned.
  16. Participate in staff meetings, training workshops, and, community meetings as assigned.

QUALIFICATIONS AND SKILLS

  1. Masters Degree in social work, health, counseling, or other related field.
  2. At least 2 years nonprofit experience, housing experience preferred.
  3. Experience working with people in crisis to access public benefits.
  4. Knowledge of San Francisco specialized services, including homeless, HIV/AIDS, substance use, mental health and LGBTQ services.
  5. Must have strong administrative skills that include computer and office skills.
  6. Clear and concise verbal, written and presentation skills. Strong interpersonal skills.
  7. Strong organizational and time management skills that includes creativity when planning and accomplishing a variety of goals and be results-oriented.

PEER NAVIGATOR

POSITION SUMMARY:  Under the direction of the Clinical Supervisor, the Peer Navigator is primarily responsible for ensuring smooth, timely and accurate intake of our members; using established standards, business rules, and policies as guidance, the Peer Navigator will act as the AHA’s first point of contact for all new and existing members, will assist members in accessing community linkages by accompanying them to appointments, and accompanying Housing Advocates with home visits. Additionally, the Peer Navigator will interface with all colleagues on the AHA Team, maintain member confidentiality, and, promote a sense of pride and professionalism at all times.
